      Position:
      Responsible for providing direct social work services to patients and their families as a part of the multidisciplinary team with most emphases on completing Psychosocial Assessments, facilitating psycho-educational training, family meetings, discharge planning, assisting in program development, and developing, implementing and reviewing Treatment Plans. Accepts direction from the Program Director.

      Essential Functions:
      
      • 
        
      • Complete psychosocial assessments on admitting patients
        
      • Initiates discharge planning at admission and sees through until time of discharge
        
      • Participates in the implementation of the treatment plan and carries out treatment as ordered
        
      • Provides education and support to families and community partners
        
      • Runs group therapy using facility evidenced based curriculum
        
      • Functions as part of the Interdisciplinary Treatment team
        
        Requirements:
        
        • 
          
        • Master's degree in Social Work from a CSWE accredited program required
          
        • LPCC, LMFT experience welcome.
          
        • Licensed as a Social Worker as required by the State of Ohio.
          
        • Experience in acute, behavioral health hospitals.
          
        • Knowledge of community resources including Medicare/Medicaid, county, state, and private social services agencies, and eligibility requirements.
          
        • Understands behavioral health and the services provided to the patient through a multidisciplinary team.
