to day as a Physical Therapist We are seeking a Part?Time or PRN
Physical Therapist (PT) to join our Home Health team serving
patients on the West Side of Cleveland . The Physical Therapist is
responsible for evaluating patient needs, developing and
implementing plans of care, and providing skilled physical therapy
services in accordance with physician orders and regulatory
standards. This role offers flexible scheduling and is ideal for
clinicians seeking part?time hours or PRN work while making a
meaningful impact within their local community. Would you be a
great Physical Therapist? To thrive as a Part-Time Physical
Therapist at VNA of Ohio Home Health Operations, LLC, candidates
must possess a variety of essential skills and qualifications. A
Bachelor's or Doctorate degree in Physical Therapy (DPT) from an
accredited program is required, alongside an active state licensure
as a Physical Therapist. A minimum of two years of experience in
physical therapy, ideally in home health or patient-centered care
settings, is necessary to ensure effective treatment. Successful
candidates will demonstrate a strong capacity for patient-centered
care, developing personalized treatment plans that enhance mobility
and quality of life. Clinical expertise in evidence-based practices
and compliance with home health regulations is vital. Exceptional
communication skills are essential for building trust with patients
and collaborating with interdisciplinary teams. The ability to
adapt care plans based on evolving patient needs, coupled with
effective time management and proficiency in electronic health
record (EHR) systems, will ensure efficient patient care and
documentation. Familiarity with Medicare and Medicaid guidelines is
also important for regulatory compliance. Qualifications: Master's
or Doctorate degree in Physical Therapy from an accredited program.
Current Ohio Physical Therapist (PT) license in good standing.
Valid driver's license with reliable transportation. Ability to
travel throughout the West Side of Cleveland service area. Strong
clinical assessment, documentation, and communication skills.
Ability to work independently in a home health setting. Home health
