Monday-Friday Position Summary The Hospice RN Case Manager is
responsible for modeling the 3 Company values of Compassion,

position is responsible for upholding the Code of Ethical Conduct
and for promoting positive working relationships within the
company, among all departments, and all external stakeholders. The
RN Case Manager is responsible to the Clinical Manager and
coordinates care for an assigned caseload of patients/families. The
RN Case Manager is committed to the philosophy, goals, and
objectives of the organization to support quality and safe patient
care practices. Position Specific Responsibilities Develops,
maintains and implements written nursing plan of care for the
assigned caseload of patients/families which address identified
needs and provide continuity of care. Assists other team members or
volunteers in planning care to meet the physical, emotional,
spiritual and other needs of the patient and family. Provides
nursing care for the patient/family through the assessment,
evaluation, planning and counseling of patients/families. Teaches,
supervises, and evaluates care provided by volunteers, home health
aides and primary caregivers. Re-evaluates the needs of the
patient/family at regularly scheduled intervals and modifies the
plan of care in accordance with patient/family changing needs, and
input from the team members which is consistent with protocol and
suggested orders. Consults with the patient's physician when
necessary. Updates the physician on a regularly scheduled basis
regarding patient’s condition and care needs. Provides a written
summary of patient care when a patient is admitted to an extended
or acute care facility to ensure the provision of continuity of
needed care. Prepares and educates the patient/family on the
end-of-life process as evidenced by observation and/or
documentation. Establishes and maintains appropriate records of
patient care management as required by company policy and
guidelines and that adhere to the standard of nursing practice.
Serves as a consultant to other professionals or volunteers as
requested or as need as identified. Coordinates and facilitates
patient care by utilizing existing agencies/services when
appropriate for example, for lab services, pharmacy, or
transportation needs. Serves as a resource to other nurses in
planning or presenting patient care conferences. Plans and
participates in in-services and continuing education programs.
Actively participates, contributes and attends required meetings.
Identifies personal and educational needs via workshops, seminars,
self-study, etc. and keeps current professionally. Identifies
educational needs and ensures that knowledge base is present before
proceeding with implementation of care. Assists with the
development, implementation and evaluation of Performance
Improvement indicators relative to the assigned programs.
Responsible for addressing indices out of compliance. Performs
Triage (on-call) duties as rotating and/or mandatory basis. Other
duties as assigned by Hospice Clinical Leadership. Education and/or
Experience Graduate from an approved school of Registered Nursing.
A bachelor’s degree in nursing preferred (BSN). Minimum of two (2)
years’ experience in Medical-Surgical, Oncology, Hospice or
community health nursing is required. Skills Mathematical Skills:
Ability to add, subtract, multiply, and divide in all units of
measure, using whole numbers, common fractions, and decimals.
Ability to compute rate, ratio, and percentage. Language Skills:
Ability to read, analyze, and interpret general business
periodicals, professional journals, technical procedures, or
governmental regulations. Ability to write reports, business
correspondence, and procedure manuals. Ability to effectively
present information and respond to questions from leaders, team
members, investors, and external parties. Excellent written and
verbal communications. Other Skills and Abilities: Ability to
understand, read, write, and speak English. Articulates and
embraces integrated healthcare at home philosophy. Certifications,
Licenses, and Registrations Current registration in the State of
Ohio. Obtains/maintains BLS/CPR recertification Physical Demands
and Work Environment: The demands of this role necessitate a team
member to effectively perform essential functions. Adaptations can
be made to accommodate team members with disabilities. Regular
standing, walking, and manual dexterity are fundamental, along with
the ability to lift and move objects up to 50 pounds. Visual acuity
requirements include close and distance vision, color and
peripheral vision, depth perception, and the ability to adjust
focus. In a healthcare setting, exposure to bodily fluids,
infectious diseases, and conditions typical to the field is
expected. Routine use of standard medical equipment and tools
associated with clinical care is essential. This description
