Provides in-home and/or telehealth treatment services to a caseload
of individuals aged 55 and over who have a mental health diagnosis.
Essential Duties and Responsibilities The following description is
a general representation of the key duties and responsibilities of
the position: Responsible for providing clinically appropriate
individual psychotherapy services to assigned clients. Performs
diagnostic assessments/reassessments as assigned. Engages clients
in the development of annual treatment goals, clinical reviews and
completion of all other documentation related to the client’s
treatment. Provides support to Day Treatment Group as facilitator
on an?as-needed basis. Maintains communication with clients,
caregivers, and helping professionals/agencies serving the client
through phone calls, written communications, and in-person
meetings. Has a working knowledge of therapeutic modalities, is
able to apply them to meet the needs of individual clients.
Provides necessary direct service hours to meet the performance
standards set by the Behavioral Health Services Department.
Completes and submits all documentation and client data in a timely
manner according to department/agency protocols and funder
requirements. Provides field instruction and/or training
supervision for students and dependently licensed employees, on an
as-needed basis. Other Functions and Responsibilities: Maintains
documentation of client records according to agency, regulatory and
funding?requirements, including but not limited to CARF, Ohio Dept.
of Behavioral Health, Ohio Dept. of Medicaid, and the Cuyahoga
County ADAMHS Board. Provides information and referrals to other
agency home and community-based service (HCBS) programs as
appropriate. Works to maintain good working relationships with
co-workers, management as well as other agency program and service
staff. Attends agency meetings as required. Requirements: Specific
knowledge, training or skills required to perform the duties of
this position including specific courses, training programs or
required certifications. The requirements listed below are
representative of the knowledge, skills and abilities required for
the position. Master’s degree in social work or clinical
counseling. Current and unrestricted Ohio licensure an LISW, LPCC,
LISW-S or LPCC-S. Demonstrated knowledge of mental illness [as
described by the Diagnostic and Statistical Manual V of the
American Psychiatric Association]. Must have a current valid Ohio
driver’s license and automobile insurance and access to personal
transportation. Must have excellent verbal, written and electronic
communication skills. Demonstrated proficiency in use of computer
programs (Microsoft Outlook and Office). Demonstrated proficiency
in use of electronic health records. Strong planning and
organization skills. Understanding of complex housing needs.
Attention to detail and accuracy are essential, as are abilities to
prioritize work and consistently meet deadlines Previous Experience
that is necessary to qualify for this position: Three years’
experience in social service field is preferred. Essential Physical

office environment. This role routinely uses standard office
equipment such as computers, phones, photocopiers, filing cabinets
and fax machines. While performing the duties of this job, the
employee is regularly required to, stand, sit; talk, hear, reach,
stoop, kneel, and use hands and fingers to operate a computer,
keyboard and telephone. Specific vision abilities required by this
job include close vision requirements due to computer work and
ability to drive personal vehicle. Light to moderate lifting is
required, up to 25 pounds. Ability to navigate local travel.
Regular, predictable attendance is required. Moderate noise (i.e.
business office with computers, phone, and printers, light
traffic). Ability to sit at a computer terminal for an extended
period of time. Ability to navigate work-related stressors
(deadlines, etc.) Ability to drive personal vehicle to meetings and
client visits when necessary.
